Create a new form in Tally to capture emails. Add a Name field and an Email field (set as required). Under the form settings, enable email notifications so you are alerted on each submission. Click Publish and copy the shareable link or embed code.
Open Tally - 2Mailchimp
Sign up for a free Mailchimp account and create a new Audience (list). Go to Audience > Settings > Audience name and defaults to find your Audience ID — you will need this for the Zapier connection.
Open Mailchimp - 3Zapier
Create a new Zap using the pre-built Tally + Mailchimp template. Set the Trigger to Tally → New Submission, and the Action to Mailchimp → Add/Update Subscriber. Map the email field from Tally to the Mailchimp Email Address field, then turn on your Zap.
